Free Kick Glitch

This happened in MFN 15

Two issues on this play:
1. Why didn't the QB throw the ball away? He had plenty of time to get rid of it instead of taking a safety.
2. After the safety, the free kick never happens and DAL gets the ball at the OPP 20.

That is really strange. Has anyone else seen this happen, and if so, would you please post the play to this thread? I need to figure out what the common denominator is to try to identify why the ball is going dead before the play starts.
